Thank you for considering a puppy from Doodles & Doxies!

We know many families have two working adults and we don’t expect anyone to be a super hero and quit a day job just to be able to take home a puppy. Every puppy family has different needs, and this information form helps us identify who needs what information and what homes might make the best fit. If you’re a veteran puppy owner, we don’t want to waste your time regaling things you already know; by the same token, if this is your first puppy, we want to make sure you leave prepared for what life with a puppy may bring.

Acknowledgements

For the following statements, "You" and "Your" refer the head of the guardian home and/or members of your household. "We" and "us" refer to Doodles & Doxies.

Dam Relinquishment(Required)

I agree to the dam relinquishment policy.

If you have a guardian home female, you will be required to relinquish her to us for a period of 8+ weeks for every litter she has. This could be up to twice a year. You and/or your family must be emotionally prepared to part with the dog for this length of time. We will do our best to extend visitation privileges to guardian families, but we cannot promise availability.

Dam Heat Cycle Management(Required)

I agree to the dam heat cycle management policy.

If you have a breeding female, you must handle her heat cycles. Heat cycles can be messy. We are here to help when the time comes, but you must be knowledgeable of the specific care necessary.

Sire Relinquishment(Required)

I agree to the sire relinquishment policy.

If you have a male dog, you will be responsible for timely transfer to us and/or the female he will be visiting at the time of breeding. You will not be required to assist in the breeding.

Sire Behavior(Required)

I agree to accept/manage sire behavior.

If you have a breeding male, he may be prone to humping and/or marking.

Testing(Required)

I agree to the testing policy.

You may be required to assist us with health, coat, and color testing of our dogs. All testing is at the expense of the breeder. Some tests may be sent to you by mail, which you must submit by mail in a timely manner. Other tests require a vet, either ours or your local animal hospital.

Breeding(Required)

I agree to the breeding decisions policy.

You relinquish any and all decisions on the breeding life of the dog to us. You must prevent breeding activity not explicitly sanctioned by us, and you must support our breeding processes and decisions.

Reporting/Communications(Required)

I agree to the reporting/communications policy.

You must be willing to send us frequent photo, video, and text updates on the dog and its health, temperament, and coat.

Guardianship Program Defense & Dam/Sire Safety(Required)

I agree to defend the guardianship and breeding program.

Please be aware that many people are opposed to breeding dogs in general. Should you be challenged by others who disagree with breeding, you must be prepared to defend your dog and/or our program, and ensure the safety of your canine charge(s) .
